To register at Quay Lane Surgery you need to be living within our practice catchment area. We do not accept out of area patients.

To register you need to dowload, print, complete and return the relevant forms below. You need to be registered before your first appointment so please make sure you return them as soon as you move to our area. You will require your NHS number - if you do not know what it is then please follow the link below or obtain from your previous surgery.

A new service is now live to help find your NHS number. This service is for anyone living in England who has forgotten or does not know their NHS number. You can also use this service on behalf of someone else where the name, date of birth and registered home postcode is known. You can opt for the number to be sent to you by text, email or letter.

VISITORS (temporary patients)
If you are staying in our area and need to be seen then we request that you contact your own GP. All practices are now offering telephone and video consultations and will be able to consult you via one of these methods. If you need a prescription then they will be able to arrange for one to be sent electronically to a local pharmacy of your choice.
